Flexible Spending Accounts
SureWest recognizes that employee
benefits are an important part of the compensation package. Accordingly,
we have endeavored to assemble a premium benefits package for our
employees.
We offer a flexible health benefits package including
medical, dental, vision, life insurance, long-term disability, KSOP
and Flexible Spending Accounts. SureWest also offers premier time
off and retirement benefits.
Below is a summary of benefits offered to employees
of SureWest and its subsidiaries. Additional details
are provided to employees when they are hired.
Medical, Dental and Vision Plans
SureWest offers four options for medical plans including Kaiser,
Pacificare, PPO Plan (Administered by Delta Health Systems) and
the Catastrophic Medical Plan (also administered by Delta Health
Systems).
SureWest’s dental plan is an indemnity plan,
administered by Delta Health Systems. You may visit the dentist
of your choice. Our coverage includes preventative, basic, major
and orthodontic services.
SureWest offers vision insurance through Medical
Eye Services. Our vision plan provides coverage for eye exams, lenses
and frames, or contact lenses.
Life Insurance
The Company pays one times the annual earnings for all full-time,
non-management employees. For management employees, the Company
pays two times the annual earnings.
All full-time employees may purchase up to an additional
four times annual earnings in supplemental life insurance. You may
also purchase life insurance for your spouse and/or dependent children.
For all part-time employees (working 20-29 hours
per week), the Company pays for $10,000 in basic life insurance
coverage.
Long-Term Disability (LTD)
All regular full-time employees are eligible for Long-Term Disability
coverage on the first of the month following one-year of service.
All full-time employees may purchase additional Long-Term Disability
Insurance up to 66 2/3% of their monthly salary (for a maximum benefit
of $10,000 per month).
KSOP (Employee Stock Ownership and 401 (k) Plan)
All regular employees may contribute from 1% to 50% of their pay
on a pre-tax or after-tax basis beginning on the first day of the month following
the month hired. After one year of service, the Company will contribute
dollar-for-dollar, up to 6% of your contribution.
You are immediately 100% vested in your own contributions,
earnings in your account, and Company matching contributions.
Flexible Spending Accounts
The Company offers Flexible Spending Accounts for dependent care
expenses and certain health care expenses not covered by insurance.
Employees are eligible to participate in the Flexible Spending Accounts
on the first of the month following 6-months of employment.
Flexible Spending Accounts allow you to direct
part of your pay, on a pre-tax basis, into a special account(s)
that can be used throughout the year to reimburse yourself for certain
out-of-pocket health care and dependent care expenses.
