This document is notification of the offering of Nokia equipped

REASON:

This equipment provides for ADSL services and interconnection.

GENERAL SPECIFICATIONS FOR INTERCONNECTION:

• The DSL end-user must be located within 17,500 feet of a SureWest Telephone enabled DSLAM site. The line must be qualified to carry the DSL signal.
• DSL requires compatible customer provided equipment (CPE) at the customer location. This equipment must be compatible with the DSLAM equipment.
• A NIC (Network Interface Card) or other compatible interface is required in the customer computer.
• An Internet Service Provider must be selected. This ISP must be connected to the SureWest Telephone DSL Network.
• An access line terminated at the customer's site may not be required.
